PURPOSE: Homeless and at risk individuals received assistance with employment skills and in meeting daily needs for food and shelter

PURPOSE: Provide one-stop service center for low-income households living in the Eugene community. Services will include case management, food, shelter, information and referral and other services as needed

Community Service Center/South Lane
PURPOSE: Provide one-stop service center for low-income households living in the Cottage Grove/South Lane communities. Services will include short term case management, shelter, food boxes, information and referral, energy assistance, and other services as needed.
OUTCOME: Immediate needs were fulfilled by enabling low-income households to stabilize their living situation

Day & Night Center for Families
PURPOSE: Provide low-income, homeless or at risk of become homeless families with children emergency services and provisions including food, day/night shelter, clothing, laundry, kitchen facilities, preschool, phones, transportation, and information and referral.
OUTCOME: Homeless households receive assistance in meeting daily needs and to stabilize their living situation
Family Shelter/Eugene
PURPOSE: Provide homeless families with children with shelter, crisis intervention and supportive services at shelter located with in Eugene city limits.
OUTCOME: Homeless households stabilize their living situation and move toward greater level of independence
Homeless Youth Shelter & Intervention
PURPOSE: Provide a continuum of services for homeless, runaway, and at-risk youth including crisis interventions, family reconciliation, shelter, case
management, substance abuse support services and health and safety education
OUTCOME: Youth will reduce or eliminate behaviors that present barriers to meeting housing, employment, and education goals
Parent Education (Open Doors) (CoC)
PURPOSE: Provide parent education and on-going support group services for parents participating in the HUD Open Doors program
OUTCOME: Open Doors families will enhance their skills and receive ongoing support for improving relationships with their children
Homeless and At Risk Youth Services
PURPOSE: Assist at risk and homeless youth make positive changes in their lives by providing services, resources, skills, and information to promote empowerment

Hunger Relief/Food Bank
PURPOSE: Solicit, collect, warehouse, prepare, transport, record, manage, purchase, and allocate food to support the needs of Lane County Residents
OUTCOME: Reduce hunger in Lane County.
Hunger Relief/Meals on Wheels
PURPOSE: Provide meals to home-bound senior citizens living in the metro and rural areas of Lane County
OUTCOME: Elderly citizens were prevented from experiencing malnutrition, enabling them to remain independent for a longer period of time

PURPOSE: To provide food boxes for low-income households living in Oakridge

HEALTH
Specialized Case Management Services for Adults/Lead Agency
PURPOSE: Identify people in the community who are continually in crisis, using an exorbitant amount of services and showing no signs of improvement. Agencies participate in a consortium to try new approaches, assign lead managers and develop community plans
OUTCOME: Individuals in crisis were stabilized through the persistent and ongoing contact by White Bird staff
Dental Clinic
PURPOSE: Provide reduced or free cost urgent ongoing and preventative dentistry to low-income children and adults. The targeted population includes those who are unemployed, uninsured, disabled, and homeless. Their common characteristics include marginal self-sufficiency
OUTCOME: Low-income households are provided with dental care
Medical Clinic
PURPOSE: Provide free and sliding-scale primary health care services for low-income, uninsured households in Lane County. Services include exams, basic lab work, and medications.
OUTCOME: Low-income households are provided with medical care
Homeless Health Care
PURPOSE: Provide free comprehensive health care to homeless people in the Eugene/Springfield area in order to meet their basic health needs and to remove barriers to their attainment of stable living conditions
OUTCOME: Homeless individuals received health care services in order to promote stability and improve their living conditions
Youth Substance Abuse Coordination
PURPOSE: Provide substance abuse services for homeless or at risk youth
OUTCOME: Reduce or eliminate substance abuse and other behaviors that present barriers to meeting housing, employment, and education goals among youth
Safe Haven Homeless Outreach/Intervention
PURPOSE: Connect chronically homeless mentally ill individuals with Safe Haven Shankle facility and other community support services
OUTCOME: Mentally ill chronically homeless individuals are able to obtain housing and connect with services to address their needs and stabilize their living situation

OUTCOME: Developmentally delayed households will obtain and maintain a safe and stable home while reversing a pattern of homelessness

PURPOSE: To reduce chronic homelessness for adults with severe persistent mental illness by providing housing, support services and day shelter services
OUTCOME: Chronic homeless adults with severe persistent mental illness are able to stabilize their living situation by increasing their household incomes and developing the skills necessary to live semi-independently

CHILD AND YOUTH DEVELOPMENT
Specialized Supportive Services for Children with Disabilities
PURPOSE: Provides essential supports and assistance to families who have children with disabilities. The program helps families negotiate the maze of services and to create, design and tailor a comprehensive support plan to ensure that a child develops to his/her fullest potential.
OUTCOME: Reduces the likelihood of children with disabilities requiring expensive and crisis based services
Families with Special Needs Program
PURPOSE: Provides nurturing and stimulating preschool services for children between the ages of two and five, born with average to above average intelligence, whose parents have cognitive limitations. The program also provides services designed to help diminish family stress, confusion, frustration and a sense of isolation
OUTCOME: Children whose parents have cognitive limitations are able to enter the public school system without requiring special education intervention.
FINANCIAL AND LEGAL COUNSELING AND EDUCATION

Mental Health Crisis Intervention
PURPOSE: Provide 24-hour seven day a week crisis response; facilitate entrance for mental health consumers into the public mental health system and facilitate referral of mental health consumers to appropriate and available community providers.
OUTCOME: Percentage of Phone, Walk-in and mobile crisis stabilization of clients served
Domestic Violence Intervention/Overnight Emergency Shelter
PURPOSE: Provides shelter and emergency services to victims of domestic violence
OUTCOME: Stabilize housing for ex-offenders and reduce the likelihood of new crimes
PARENTING EDUCATION AND SKILL DEVELOPMENT
Therapeutic Childcare and Parent Education (Eugene & Springfield)
PURPOSE: Provide family support and early intervention services to children of high risk families from the ages of 0-6. Services such as support, counseling, education, and home visits are offered
